FEMA-USFA-NFA collects this information in accordance with Public Law 93- 498.

Purpose:

This information is being collected for the primary purpose of establishing communication with National Fire Incident Reporting System (NFIRS) users and aiding in the resolution of any information technology-related issues they may report.

Routine Use:

The information on this form may be disclosed as generally permitted under 5 U.S.C. § 552a(b) of the Privacy Act of 1974, as amended. This includes using this information as necessary and authorized by the routine uses published in DHS/ALL-002 Department of Homeland Security (DHS) Mailing and Other Lists System, November 25, 2008, 73 FR 71659 and DHS/ALL-016 Correspondence Records, September 26, 2018, 83 FR 48645, and upon written request, by agreement, or as required by law.
